Service plans & reassessments (MN 144G)
The assisted-living care document — generated, not retyped.
Assisted living doesn't run on a CMS-485. It runs on a service plan under Minnesota's
Chapter 144G — and on a reassessment cadence a surveyor will ask about. Carelytic builds the
plan from the admission assessment and keeps the reassessment clock so nothing lapses.
Service Plan from the admission assessment
The MN Nursing Admission Assessment feeds an Individualized Service Plan built to §144G.70(4)(f) content. The plan is generated from what the nurse assessed — the clinician reviews and finalizes rather than starting from a blank template.
Type-driven reassessment engine
Reassessments are interval-configurable and type-driven — a two-tier cadence tied to the resident's service tier. The bed-rail safety reassessment fires on the 90-day MDH cadence; physician re-exam is tracked on its own clock. Due and overdue reassessments surface on the Compliance hub.
Admission checklist with e-signature
The admission checklist captures e-signatures and in-app photo capture — Bill of Rights, service agreement, and the required 144G disclosures signed on the device, filed to the resident's documents, no scan-and-upload dance.
MN 144G assessment set
Nursing admission, ISP, nutrition and mood screens with follow-up triggers, and a Bed Rail Safety assessment — the assessments a Minnesota assisted-living or assisted-living-with-dementia-care license actually requires.
Follow-up triggers, built in
A flagged nutrition or mood screen auto-creates the follow-up — the same trigger philosophy that closes audit gaps on the home-health side, applied to the resident's care plan.
State-configurable by design
The ruleset is data, not hard-code: Minnesota 144G ships today as a provisioning pack, and the assessment / service-plan / reassessment engine is built to be configured for other states' assisted-living rules as they're added.
